Provide timely and effective medical social work and discharge planning that encompasses initial and continuing psychosocial assessment of individuals for their medical, financial, emotional, and spiritual needs. This assessment involves the ability to counsel, interview, evaluate, organize, prioritize, communicate, document, and implement a safe comprehensive cost effective discharge plan. The plan includes referrals to appropriate healthcare providers and community resources for continuity of care and to ease the patient�s transition through the continuum of care. Provides coordination of healthcare services for patients to meet healthcare requirements in a cost-effective manner. Participates in a collaborative process with the interdisciplinary team to assess, plan, implement, coordinate, monitor and evaluate options and services to meet individual�s health care needs while promoting quality, cost-effective outcomes. Participates in the utilization review process for patients. Demonstrates knowledge of local, state, and federal regulations and guidelines in accordance with policies, procedures, mission and objectives.
POSITION DUTIES
Qualifications: EDUCATION Required: Master�s Degree in Social Work from a school accredited or approved by the Council on Social Work or Master�s Degree in Counseling or Rehabilitation. Preferred: Master�s Degree in Social Work/Counseling
WORK EXPERIENCE Preferred: Over 2 years recent acute health care experience. Behavioral health experience
OTHERS Skills Required: Leadership; communication and teaching; organizational; interpersonal; team building and collaboration; psychosocial assessment; interview; documentation; identification of potential risk situations or potential liability.
Knowledge Required: Quality improvement process, related health care practices and standards; knowledge and competency in providing care for patients in neonatal, and/or pediatric, and/or adolescent, and/or adult, and/or geriatric age groups; current practices, issues, regulations involved in transitions planning; psychosocial aspects of disease and health care; professional and regulatory standards.
Technology Required: Basic computer skills, personal technology tools, awareness of current use of technology in related field. Preferred: Advanced computer skills, technology of the specialty.
Other Required: Must be able to transport self within the service area as required. When using own vehicle must have valid Arizona drivers� license and proof of insurance that meets the minimum standards of the state of Arizona.
