Overview
Northwest Permanente is seeking a Board Certified or Board Eligible Internist, Family Practitioner, Geriatrician or Hospitalist to join our growing medical group as part of our dedicated team of "Continuum Complexivist" Physicians. This Continuum Complexivist opportunity is based in Portland, OR and will work closely with an experienced, cohesive interdisciplinary team in our core Skilled Nursing facilities (SNF). This team also provides virtual support to members of our Home Health program, including nurses, therapists and social workers. For interested, fellowship-trained Geriatricians, you have the added opportunity of teaching OHSU/ VA Geriatric fellows that rotate through the department.
We emphasize transitions of care from the hospital to the skilled nursing setting and from a skilled nursing facility to home or other settings of custodial care. Department goals include decreasing emergency room visits and re-hospitalization, as well as transitioning to palliative and hospice care, when appropriate. We provide a team approach, usually with one to two Complexivists (MD/NP) and a Kaiser Care Coordinator, that care for a spectrum of medical complexities, ranging from straight-forward, elective orthopedic surgery patients to those with multiple, complex medical problems at each facility.
You will enjoy a predominantly weekday, daytime work schedule. This includes supporting our Complexivist Call Center from home, virtually, and makes up ~20% of the role. The Complexivist Call Center does include early, day, or evening shifts and supports the home health field staff, when they are in patient's homes, as well as consults (physician consults to determine patient eligibility for SNF). You will also support our "Transition Nurse Team" for escalations of patients transitioning from recent Hospitalist discharges or SNF discharges.
Applicants should have experience with digital technologies (AI platforms, telehealth and/ or digital health as examples). If no direct experience, then you will need to demonstrate both flexibility and adaptability to incorporate these into your practice as we continue to evolve our virtual care model.
