The Pain Medicine Specialist practices a majority of the time in the Roseburg, VA Health Care System (RVAHCS) medicine clinics, with an integral role as a member of a multidisciplinary team to manage and assist other providers in the management of veterans with chronic and acute pain. To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. The Pain Medicine Specialist (Physician) is responsible for providing and assisting with the coordination of care for chronic pain patients enrolled with RVAHCS. The incumbent provides consultation service locally and regionally for providers working with complex chronic pain care, and provides education to patients and to providers regarding safe and sustainable chronic pain care and effective functional restoration of patients suffering with chronic pain. Clinical Duties (but are not limited to): Completes detailed history and physical exams, as well as health inventories, orders and interprets diagnostic studies and prescribes appropriate medication and other treatment modalities. Develops, evaluates and manages medical treatment plans intended to achieve defined outcomes that improve the patient's quality of life as part of a coordinated multidisciplinary treatment team focused on safe and sustainable chronic pain care and functional restoration of patients living with chronic pain. Provides care management that includes timely, thorough, and efficient documentation & coordination. Orders, performs, reviews, analyzes and follows up on appropriate diagnostic/laboratory test results, including any registries necessary to monitor and support the patient's medication therapy, and communicates findings to patients and other providers, including additional surveillance and/or testing requirements necessary as per VA policy. Initiates consults as appropriate, follows the treatment plan of care, coordinates and discusses plan with other care providers and specialists as needed. Establishes and maintains contact with other care providers, specialists, pharmacy and ancillary staff as necessary within the facility and the private sector. Review non-VA records and updates electronic health records with outside records. Provides care in outpatient, inpatient and residential program settings and participates in the physician call pool. Performs suicide and opioid risk screenings and assessments. Treats inpatients and outpatients with chronic pain and provides opioid reduction, harm reduction, and opioid safety/Medication Assisted Treatment (MAT) services as needed. Provides patient care via multiple modalities, including in person, video, and telephone visits on an individual or group basis, as well as electronic consults for referred patients for comprehensive pain management. Other clinical duties as determined by the Supervisor. Administrative Support Duties (but are not limited to): Serves as the primary liaison for the full implementation of the Stepped Care Model for Pain Management (SCM-PM). Partners with the Facility PMOP Coordinator to foster required team composition and intended function of the CARA-mandated interdisciplinary Pain Management Teams (PMTs)/Pain Clinics. Supports implementation of the Stepped Care Model for Opioid Use Disorder within Pain Management Teams/Pain Specialty Clinics. Facilitates Facility access to tertiary pain center within the VISN including a CARF-accredited Interdisciplinary Pain Rehabilitation Program. Works in collaboration with the Facility-level partners such as the Facility PACT Pain Champion, Facility PMOP Coordinator, Suicide Prevention Team, Primary Care/Mental Health Integration, and Mental Health Leaders to support suicide prevention efforts and awareness in Veterans experiencing pain.
