Schedule: Full Time

Shift: Monday to Friday 8am to 5pm

Location: Team Green:  This position will be conducting patient home visits in the Kissimmee, Celebration, St. Cloud, and Osceola County area which includes but is not limited to the following zip codes:  32836, 32821, 32830, 32837, 32824, 32827, 32832, 34773, 34771, 34772, 34769, 34744, 34743, 34741, 34746, 34758, 33896, 34759, 34747

The role you’ll contribute:

The Physical Therapist Assistant is responsible for Carries out various physical therapy procedures under the supervision of a Physical Therapist. State licensure required. Provides treatment to relieve pain and to develop or restore function as ordered on Plan of Care. Provides accurate documentation. Teaches use of assistive devices and equipment to patients and caregivers.

• Observes, records and reports to physician any untoward reactions to treatment.
• Instructs and supervises family members to provide continuous therapy and home treatment plan. 
• Maintains open communication with case manager.
• Submits daily log and charge sheets to Home Health office.
• Maintains communication with scheduler regarding caseload limitations.
• Participates in case conferences.
• Maintains open communication with primary Therapist.
• Possesses excellent time management skills.
• Meets agency’s expectations regarding productivity.
• Provides in-service training to staff as requested.

Qualifications - External

The expertise and experiences you’ll need to succeed:

• Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association
• Currently licensed and registered in the state of Florida.
• Basic Life Support Certified 
• Valid Florida driver's license, safe driving record and reliable transportation.
